Signature Membership


Signature membership provides the foundation of what Praxis was built to offer — a personal physician who knows your history, handles the full range of what medicine requires, and is there when something changes.

Your Physician, Available When It Matters.


If a Signature member is ever seen in an emergency department or admitted to a hospital, your Praxis physician will speak directly with your treating team — helping coordinate care, provide clinical history, and ensure continuity across care settings. You will never navigate a hospitalization alone.

Hospital & Emergency Department Coordination


Semiannual Health Evaluation

Twice per year, Signature members participate in an extended, dedicated physician visit — a scheduled, unhurried evaluation reviewing current health status, preventive priorities, medications, and long-term goals. This is not a routine check-in. It is a deliberate, proactive investment in your health outside of acute concerns.


Expedited Diagnostics & Specialist Coordination

When imaging, laboratory testing, or specialist involvement is needed, your physician coordinates directly — leveraging personal relationships to accelerate access and ensure results are reviewed and explained personally.


Members reach their physician directly by phone or text for urgent medical concerns — any time of day or night. There is no answering service, no nurse line, and no after-hours recording.

Most care is delivered through secure telehealth for speed and convenience. When in-person evaluation is clinically appropriate, house calls bring physician care directly to you. Signature membership includes up to four in-home visits per membership year.

Priority membership is designed for patients who want their physician woven more completely into their life — not only available for urgent concerns, but accessible for the routine, the complex, and everything in between, on their schedule and at their pace.

Priority Scheduling & Expedited Access


Unlimited In-Home Visits

Priority membership removes the annual limit on house calls. When in-person evaluation is clinically appropriate, home visits are available without restriction — reflecting the depth of availability this tier is designed to provide.


Advanced Preventive Screening

Priority membership includes one advanced preventive screening study per membership year — such as coronary artery calcium scoring or other modalities selected at the physician's clinical discretion based on individual risk and goals.


Priority members have direct physician access at any hour, for any reason. This means that a video call to discuss a routine medication question, a travel health plan, or an upcoming procedure can happen on your schedule — including evenings, weekends, and off-hours — without waiting for the next available appointment. Availability extends beyond acute urgency to the full scope of your medical needs.

Expanded Availability — Beyond Urgent Concerns


As with Signature membership, your Praxis physician will communicate directly with any emergency or inpatient team if you are ever hospitalized — providing clinical history, coordinating care, and remaining actively involved throughout your care episode.

Hospital & Emergency Department Coordination


Semiannual Priority Health Evaluations

Twice per year, Priority members participate in an extended, individually architected health evaluation — designed not around a standard panel, but around you. Drawing on your personal history, family risk factors, lifestyle, occupational demands, and health goals, your physician determines the most clinically meaningful diagnostic approach for each evaluation.

These visits typically include advanced laboratory analysis selected specifically for your risk profile, cardiovascular and metabolic assessment, and an extended physician consultation to review findings in depth and refine your long-term health strategy. Bi-annual specimen collection is coordinated through a white-glove mobile phlebotomy service when available, eliminating the need for laboratory visits entirely.


Comprehensive Care Coordination

For complex or evolving medical situations, Priority members receive advanced, hands-on physician coordination — direct provider-to-provider communication, proactive management of specialist relationships, and continuous involvement across care settings.

Child Membership

Child memberships are available as an add-on to an active adult Signature or Priority membership. Designed for generally healthy children and adolescents, child membership provides direct physician access, medical guidance and triage, acute care evaluation, and coordination with pediatric specialists when needed.

Child membership eligibility is subject to physician review to ensure Praxis is the appropriate care setting for each child's needs. Full details, including eligibility criteria, are available on our FAQ page.

Medicare Beneficiaries

Praxis Private Physician participates in the Medicare program. Medicare beneficiaries are welcome as members, and covered medical services — including evaluation and management visits, telehealth encounters, and medically necessary home visits — are billed to Medicare as required by federal law.

The membership fee covers enhanced access, care coordination, and physician availability services that are separate from and not reimbursable by Medicare. These include direct physician communication, extended consultations, care navigation, and the concierge-level services that define the Praxis membership experience.

Medicare beneficiaries will receive a separate Medicare Beneficiary Addendum as part of the enrollment process, which outlines these distinctions in detail. Membership does not affect or limit any rights you have under Medicare.

A Note on Membership Availability

Praxis maintains a deliberately limited patient panel to preserve the quality of access and care that membership is designed to provide. Enrollment is accepted on a limited basis.
